Assisted Living Record publishes a compilation of public Florida AHCA facility records. The facility data on this site is public-record information and contains no personal information about you. This policy concerns only the information you choose to submit through a contact request.
What we collect
When you submit a contact request we collect the fields you enter — your name, phone number, email address, the nature of the concern, your relationship to the resident, and any description you provide — together with your consent choices and a record of the consent (including a TrustedForm certificate). We also record which facility record the request came from.
What we never ask for
We never ask for your Social Security number, date of birth, home address, or medical records through this site. Please do not submit them.
How it is used
If you ask to be contacted, your request may be shared with a participating attorney so that attorney can reach you about your concern. Assisted Living Record is a data bureau and advertising service, not a law firm; it does not provide legal advice and recommends no attorney. Automated phone-call or text-message contact occurs only if you separately opt in, and that consent is never required to submit a request.
Retention and contact
Requests are retained only as long as needed to act on them and to meet legal and record-keeping obligations. To ask about or remove your information, write to records@assistedlivingrecord.com.
