Methodology

Assisted Living Record is a faithful compiler of a public government record. Every count on a facility page is reproduced from the Florida Agency for Health Care Administration (AHCA) survey data. We do not score, rank, grade, or characterize any facility, operator, or person. The numbers are the state’s; the page only arranges them.

The underlying data is the Florida AHCA public assisted-living facility licensure and survey record, including deficiency-class counts, substantiated complaints, sanctions, fines, and licensure status. The record is published by AHCA under a public-domain dedication. Each facility page names the date the record was compiled and a content hash of the snapshot it was drawn from.

Deficiency classes, as defined by Florida

AHCA classifies survey deficiencies by severity. The definitions below are the state’s own classifications, reproduced so a reader can interpret the counts; they are not our assessment.

Class I
A deficiency that the state finds presents an imminent danger, or a substantial probability of death or serious physical or emotional harm, to a resident.
Class II
A deficiency that the state finds directly threatens the physical or emotional health, safety, or security of residents.
Class III
A deficiency that the state finds indirectly or potentially threatens the physical or emotional health, safety, or security of residents.
Class IV
A deficiency that the state finds relates to required reports, forms, or documents and does not have the potential to negatively affect residents.

Which records are published

A facility record is surfaced for indexing only when it carries a documented adverse signal in the state record — a Class I or Class II deficiency, a substantiated complaint, or a sanction. This is a test of data presence, not of severity or commercial value: a record either carries one of these signals or it does not. Records without such a signal still render in full, but are not indexed.

Corrections

If a published count does not match the current state record, write to records@assistedlivingrecord.com. Because every figure is reproduced from AHCA, the authoritative correction is a change in the state record itself; we re-compile from the source.
