About Assisted Living Record
Assisted Living Record is an independent data bureau. We compile the public Florida Agency for Health Care Administration (AHCA) record for assisted-living facilities into a single, readable document per facility. Our work is compilation, not judgement: we reproduce what the state documented and arrange it so a reader can find it.
What we publish
For each facility we surface the AHCA survey record — deficiency classes (I–IV), substantiated complaints, sanctions and final orders, fines, and licensure status — alongside the operator of record. Every figure is reproduced from the state record and is dated to the snapshot it was drawn from. We apply no score, ranking, grade, or characterization.
What we are not
ALF Data is a data bureau and advertising service — not a law firm. It provides no legal advice and recommends no attorney. Reading this site creates no attorney-client relationship. We are not affiliated with, endorsed by, or sponsored by AHCA or any government agency; the data is public-record information published by the state.
